Policy Maintenance
Update your policy any time.
We know that things change. Keeping your contact information on your policy up to date makes the claim process much easier.
You can easily request a copy of your policy or make a variety of changes:
- The policyholder name
- Your address
- The policyholder Social Security Number
- Add or change policy beneficiaries
Simply fill out the appropriate form and send it back. A $10 fee per duplicate policy may apply.
