Accident Claims
Accident coverage is valid 24/7, on or off the job. You must submit your claim 12 months from the date of injury. You can claim as you go – you don’t have to wait for all your medical treatments to be completed before you open a case and file a claim. You can file a claim if you are receiving Worker’s Compensation Benefits.
- Complete an Accident Claim Form for each insured person who sought medical attention
- Provide medical documentation, visits, physical therapy notes, etc.
- If you are admitted to the hospital, provide admission and discharge dates
- Provide a police report if available
- Provide an Operative or Surgical Report if available
- Once a year, you may claim $50 for a wellness exam, no injury required!
Disability Claims
Short-term disability policies have a 14-day waiting period; long term has a 90-day waiting period. Your document should be dated the day your disability starts or thereafter. Claim forms predated prior to the start of your disability will not be honored. You cannot receive these benefits if you are on a Worker’s Compensation claim.
- Complete a Disability Claim Form. Sections I, II, and III must be completed before your claim can be processed.
- Complete a Culinary Union Health Fund Loss of Time claim form.
- Your employer must approve your FMLA/LOA. Otherwise, they will not complete your disability claim forms.
- You are responsible for following up with your claim.
- Have your physcian complete the Continuation of Disability form if your disability is extended.
- Ink physician signatures are required on documents. Electronic signatures are not accepted.
Critical Illness Claims
If you are diagnosed with cancer, heart attack, stroke, renal failure, or an organ transplant, file a claim. You must file within 12 months of your initial diagnosis.
- Complete the appropriate critical illness claim form.
- It's very important for your physician to complete the physician section before submitting the claim.
- If your claim is due to a cancer diagnosis, be sure your physician provides the pathology report.
- Once a year, you may claim $50 for a wellness exam, no illness required!
Life Insurance Claims
When your spouse or loved one passes unexpectedly, life insurance takes one worry off the table. This form starts the process.
- Attach an original, certified copy of the death certificate.
- It's also important to have a copy of the policy itself for submission if available.
- Choose your type of payout on the form.

Premium Payments While On Leave
If you are currently paying your policies monthly through your personal bank account via EFT, those premium payments will continue.
If your premiums are paid through automatic payroll deductions, you will need to remit monthly premiums directly to Boston Mutual Life Insurance so your policies are not affected by falling behind or unintentional lapse while you are not at work.
Pay Online
Online payment is quick and easy. Click the button below to be transferred to Boston Mutual’s online payment system.
You’ll need your policy number and the last 4 digits of your SSN. Please use CAPS LOCK when entering policy numbers. Users have reported error messages otherwise.
